Vaginal discharge is a normal and important part of vaginal health, helping to maintain balance within the vaginal environment.
Healthy discharge is typically clear or white, odourless, and varies throughout the menstrual cycle.
However, changes in discharge — including colour, consistency, or odour — may indicate an underlying imbalance or infection.
Persistent or recurrent abnormal discharge is often linked to changes in the vaginal microbiome, where protective bacteria are disrupted.
Symptoms may include: unusual vaginal discharge (white, yellow, green or grey), strong or unpleasant odour, itching, irritation, or discomfort.
Some women may also experience burning, redness, or discomfort during sex or urination.
In some cases, symptoms may improve temporarily with treatment but then return.
Recurrent or persistent discharge is often not caused by a single issue, but by underlying imbalance within the vaginal microbiome.
When protective Lactobacillus bacteria are reduced, other organisms such as Candida or bacteria associated with BV may overgrow.
Repeated use of treatments without addressing underlying drivers can lead to temporary relief followed by recurrence.
Contributing factors may include: hormonal changes, antibiotic use, sexual activity, hygiene products, and ongoing microbiome disruption.
In some cases, symptoms may be caused by more than one condition at the same time, such as a combination of thrush and bacterial imbalance.
This can make symptoms more persistent and may explain why standard treatments are not always fully effective.
Identifying the underlying pattern is important to guide targeted and appropriate treatment.
Treatment depends on the underlying cause and may include antifungal or antibacterial medications.
These treatments can be effective for symptom relief, however they do not always address why symptoms continue to recur.
Repeated treatment without further investigation may contribute to ongoing imbalance.
In my clinical practice, persistent vaginal symptoms are assessed from a microbiome-focused and whole-system perspective.
1. Comprehensive Clinical History - A detailed consultation explores symptom patterns, triggers, hormonal influences, previous treatments, and lifestyle factors.
2. Targeted Testing (when appropriate) - Testing may help identify the presence of Candida, bacterial imbalance, or mixed infections to guide treatment.
3. Targeted Treatment - This may include appropriate antifungal or antibacterial treatment alongside strategies to address contributing factors.
4. Microbiome Restoration - Support may include targeted probiotic therapy, vaginal microbiome support, and lifestyle or dietary strategies.
The goal is to restore balance, reduce recurrence, and support long-term vaginal health.
