Understanding Gardnerella: What It Means for BV, Vaginal Dysbiosis and Vaginal Health

"You've been told your swab showed Gardnerella. But what does that actually mean?"

Many women receive vaginal swab results showing Gardnerella and immediately assume they have an infection. Others are told they have bacterial vaginosis (BV) and notice Gardnerella listed on their results without understanding its role.

The reality is that Gardnerella is one of the most misunderstood bacteria in women's health.

Over the past decade, research into the vaginal microbiome has transformed our understanding of Gardnerella, bacterial vaginosis and recurrent vaginal symptoms. Rather than viewing Gardnerella as a simple infection, researchers now understand it as one part of a much larger and more complex vaginal ecosystem.

Understanding Gardnerella can help explain why some women experience recurrent BV, why symptoms sometimes continue despite treatment and why vaginal health is often more complex than simply finding one "bad bacteria."

What Is Gardnerella?

Gardnerella is a type of bacteria that can naturally exist within the vaginal microbiome.

The vaginal microbiome is the community of microorganisms that live within the vagina. It contains billions of bacteria and other microorganisms that interact with one another and help influence vaginal health.

Many women are surprised to learn that Gardnerella is not always harmful. In fact, Gardnerella can sometimes be found in women who have no symptoms and no evidence of infection.

This is an important point because the presence of Gardnerella alone does not automatically mean something is wrong.

Like many bacteria within the body, its significance depends on the overall balance of the environment in which it exists.

Understanding the Vaginal Microbiome

To understand Gardnerella properly, it helps to understand how the vaginal microbiome works.

In many healthy reproductive-age women, the vaginal microbiome is dominated by beneficial bacteria known as Lactobacillus species.

These bacteria help maintain an acidic vaginal pH through the production of lactic acid. This acidic environment supports vaginal health and helps prevent the overgrowth of certain microorganisms.

Common Lactobacillus species include Lactobacillus crispatus, Lactobacillus jensenii, Lactobacillus gasseri and Lactobacillus iners.

When Lactobacillus populations are abundant, the vaginal ecosystem is often more stable. However, when these protective bacteria decline, other microorganisms may become more abundant.

Gardnerella is one of the bacteria commonly associated with these microbiome changes.

Does Gardnerella Mean You Have Bacterial Vaginosis?

Not necessarily.

One of the most common misconceptions is that finding Gardnerella automatically means a woman has bacterial vaginosis.

While Gardnerella is strongly associated with BV, it can also be present in women who have no symptoms and no evidence of disease.

The diagnosis of bacterial vaginosis is based on the overall clinical picture rather than the presence of a single bacteria.

Healthcare professionals consider factors such as symptoms, examination findings where appropriate, vaginal pH and laboratory testing results.

This is why a swab result should always be interpreted in context rather than viewed in isolation.

What Is Bacterial Vaginosis?

Bacterial vaginosis occurs when the normal balance of bacteria within the vagina changes.

Instead of being dominated by Lactobacillus species, the vaginal microbiome becomes populated by increased numbers of other bacteria. Gardnerella is one of the most commonly identified organisms during this process.

This shift in microbial balance is known as vaginal dysbiosis.

Women with BV may experience increased vaginal discharge, a strong or fishy odour, symptoms that worsen after sex, vaginal discomfort or changes in their normal vaginal environment.

However, not all women with BV experience symptoms. Some may have significant microbiome changes without noticing any obvious problems.

Gardnerella and Vaginal Dysbiosis

Vaginal dysbiosis describes an imbalance within the vaginal microbiome.

Rather than a healthy Lactobacillus-dominant environment, the balance of microorganisms changes and other bacteria become more abundant.

Gardnerella is commonly associated with vaginal dysbiosis and is often one of several bacteria involved.

Modern research suggests that BV is rarely caused by a single microorganism acting alone. Instead, it usually involves complex interactions between multiple bacterial species within the vaginal ecosystem.

This has changed the way clinicians think about recurrent vaginal symptoms.

Rather than focusing solely on eliminating one bacteria, there is increasing recognition that the overall health and stability of the vaginal microbiome are equally important.

Why Is Gardnerella Important?

Gardnerella has attracted significant research attention because of its potential role in bacterial biofilms.

A biofilm is a structured community of bacteria that attaches to a surface and surrounds itself with a protective layer.

You can think of a biofilm as a microscopic shield that helps bacteria survive within their environment.

Researchers have found that Gardnerella may play an important role in the formation of vaginal biofilms associated with bacterial vaginosis.

Within these biofilms, bacteria can attach to the vaginal lining, interact with other microorganisms and become more difficult to eliminate.

This discovery has helped explain why recurrent BV can be challenging for some women and why symptoms sometimes return despite appropriate treatment.

Why Does Gardnerella Keep Coming Back?

Many women experience repeated episodes of bacterial vaginosis and become frustrated when symptoms continue to return.

The answer is often more complex than simply having persistent bacteria.

One possibility is that treatment successfully reduces BV-associated bacteria but does not fully restore a healthy Lactobacillus-dominant microbiome.

Without sufficient Lactobacillus bacteria, the vaginal environment may remain vulnerable to future dysbiosis.

Researchers are also investigating the role of biofilms in recurrence. If bacteria remain protected within a biofilm, symptoms may improve temporarily before returning later.

Hormonal influences, sexual activity, individual microbiome differences and other vaginal conditions may also contribute to recurrence.

For many women, recurrent BV is not caused by a single factor but by several factors interacting together.

Can Hormones Affect Gardnerella?

Hormones play a significant role in vaginal health.

Oestrogen helps support vaginal tissue health and promotes glycogen production within vaginal cells. Glycogen acts as a fuel source for Lactobacillus bacteria.

When hormone levels change, the vaginal microbiome may change as well.

This may occur during perimenopause, menopause, pregnancy, postpartum recovery, breastfeeding and with certain hormonal medications.

Changes in hormone levels can influence vaginal pH, Lactobacillus populations and overall microbiome stability.

These changes may increase susceptibility to vaginal dysbiosis in some women.

Can Sexual Activity Influence Gardnerella?

Sexual activity can influence the vaginal environment, although bacterial vaginosis is not considered a traditional sexually transmitted infection.

Research has identified associations between BV and factors such as new sexual partners, multiple sexual partners, female sexual partners and unprotected intercourse.

Semen has a naturally higher pH than the vagina and may temporarily alter the vaginal environment.

Many women notice that symptoms seem to flare after sex. This does not necessarily mean sex caused the problem, but it may contribute to changes within the vaginal ecosystem.

What About Vaginal Microbiome Testing?

Interest in vaginal microbiome testing has increased significantly in recent years.

Some advanced tests can provide detailed information about Lactobacillus species, Gardnerella abundance, bacterial diversity and other microorganisms associated with vaginal dysbiosis.

For women experiencing recurrent or complex symptoms, this information may provide additional insight into what is happening within the vaginal ecosystem.

However, microbiome testing remains an evolving area of medicine. Results should always be interpreted alongside symptoms, medical history, clinical assessment and standard investigations.

A test result alone does not determine a diagnosis or treatment plan.

The Bottom Line

Gardnerella is one of the most important bacteria associated with bacterial vaginosis and vaginal dysbiosis, but its presence alone does not necessarily mean something is wrong.

Many women naturally carry Gardnerella without symptoms or health concerns.

Problems may arise when changes within the vaginal microbiome reduce protective Lactobacillus populations and disrupt the normal balance of the vaginal ecosystem.
